Algorithmic Approaches to Coagulation Testing: What a Hematologist Should Know
This program is a Friday Satellite Symposium preceding the 57th ASH Annual Meeting. It's overall objective is to inform participants about cost-effective, diagnostically efficient test ordering strategies for evaluation of prolonged screening clotting time tests (PT, APTT), screening and esoteric testing for the patient with bleeding symptoms, lupus anticoagulant testing, laboratory evaluation of thrombophilia and thrombotic microangiopathy, and pediatric coagulation testing. Using test ordering algorithms based on data derived from the Mayo Clinic Special Coagulation Laboratory practice and the literature, we will illustrate how costly and unnecessary biochemical, phenotypic and genetic testing can be avoided for the participants’ practices while enhancing highest quality medical care.

CONFERENCE OBJECTIVESUpon completion of this activity, participants should be able to:
- Recount pathophysiologic categories of plasmatic coagulopathies causing prolonged APTT and/or PT.
- Identify effective laboratory testing strategies for evaluating prolonged APTT and/or PT.
- Efficiently and cost-effectively utilize coagulation screening tests in the evaluation of bleeding patients.
- Determine when and how to pursue esoteric testing to diagnose unusual congenital and acquired bleeding disorders.
- Discuss specific testing recommendations for lupus anticoagulant (LA) including screening, mixing and confirmatory and exclusionary coagulation testing for LA.
- Identify pre-analytical, analytical, and post-analytical variables impacting results and interpretation of LA testing.
- Summarize the variables affecting results of assays for thrombophilia
- Apply an algorithmic approach to thrombophilia testing
- Use an evidence based algorithm to discriminate between, and correctly diagnose, disorders aggregately described as thrombotic microangiopathies, enabling the selection of appropriate treatment programs.
- Identify age-related differences in coagulation parameters and recognize the variation of normal ranges with laboratory techniques to aid accurate interpretation.
