A 76-year-old gentleman presented with severe fatigue and night sweats. CBC showed macrocytic anemia (hemoglobin: 6.9 g/dL; MCV: 103 fL) and thrombocytopenia (platelet: 75 x 109/L). Differential peripheral blood counts were as noted here (Neutrophils: 34; Lymphocytes: 24; Monocytes: 10; Eosinophils: 3; Basophils: 1; Metamyelocytes: 1; Myelocytes: 2; Blasts: 25), and the bone marrow differentials were as noted (Segs and bands: 5; Myelocytes: 2; Blasts: 81; Normoblasts: 5; Monocytes: 1; Lymphocytes: 6).
