Bile acids malabsorption panel leads to IBS-D diagnosis

Situation

A 72-year-old female known to have diarrhea-predominant IBS since 2001 presented to the gastrointestinal clinic for follow-up for her symptoms. 

Results

  • Results showed 7AC4: 243 ng/mL and %CDCA + CA: 11.3%. Based on these results, her IBS-D was attributed to bile acid malabsorption.

Next steps

  • The patient was recommended to take cholestyramine 4g twice daily. One month following initiation of cholestyramine, she called to update the physician and reported that she had noticed positive improvements in her symptoms. Specifically, she was able to eat the entire meal without having to get up from the table to defecate. She also reported that her stools have been firmer and that she was not experiencing any episodes of diarrhea throughout the day.
