Internal Mayo Clinic Faculty Forms 

Please download and complete the forms that are specified in your faculty confirmation email. You will also need to fill out your disclosure in the portal online. An automated email will be sent regarding your disclosure. You may send completed forms back to the individual who requested them.

Unless Mayo Clinic has received permission to use copyrighted materials owned by someone else, or a fair use exception has been met (and there are very limited exceptions), the guidelines below should be followed:

  • Images, illustrations, and photographs (including cartoons): Images, illustrations, and photographs are each considered separate copyrighted works, and so copying of an image, illustration or photograph is not permitted without permission from the copyright owner.
  • Figures, graphs, and tables which consist of graphic representations of data: Graphical representations of factual data are not subject to copyright protection and may be copied in their entirety.
  • Headlines and titles: Headlines and titles are not subject to copyright protection and may be copied in their entirety.
  • Videos/movies/television clips/music videos: Do not publicly show videos, movies, television clips, or music videos without permission from the copyright owner(s).
  • YouTube Videos: Use only the link and do not copy them (e.g., copy to a disk) for distribution.
  • A common misconception is that copying is permissible if the source work is properly cited. While this is relevant to the academic question of whether the user has committed plagiarism, it is not relevant to the legal question of whether the user has committed copyright infringement. Put more simply, citing the source work is not a defense to a claim of copyright infringement.
